Rehabilitation and Aged Care Service

The Carl Streich Rehabilitation and Aged Care Unit is part of the Armadale Kelmscott District Memorial Hospital and provides a range of therapy
and specialist services including:

A 24 bed rehabilitation ward: The ward provides assessment of elderly
people by a multi disciplinary team of professional health workers skilled in
aged care, resulting in the implementation of a rehabilitation program,
assistance with placement in residential care or coordinated discharge
planning to provide the appropriate community support.

Aged Care Assessment Team: The Consultant Geriatrician, Social Worker
and/or Registered Nurse can provide assessment and advice regarding Home Care
Packages and residential care in a Nursing Home or Hostel.The
role of ACATs is to determine the overall care needs of frail older people
and to assist them to gain access to the most appropriate types of services.
Aged Care Assessment Teams are made up of doctors, nurses, social workers
and other health professionals who can provide a thorough assessment of care
needs and offer advice on suitable and available care options. In doing
this, ACATs comprehensively assess older people taking account of the
restorative, physical, medical, psychological, cultural and social
dimensions of their care needs. ACATs involve clients, their carers, and
service providers in the assessment and care planning process. If care at
home is agreed as being the most appropriate type of care, ACATs will refer
to local community services. If residential care is agreed as the best care
option, ACATs can approve high or low care and refer to residential aged
care facilities. ACATs are also encouraged to involve the person’s General
Practitioner (GP).-
Continence Clinic: This service is coordinated by a Clinical Nurse
Continence Adviser, with the support of a range of therapists who can offer
assessment, advice, treatment and support for adult patients with continence
problems.

Physiotherapy:
A fully equipped and staffed physiotherapy department
provides outpatient therapy for a range of patients with disabilities or
injuries, to promote maximum independence.

Occupational Therapy Home Visiting Service: Promotes safety in the
home, educates patients and where required, provides additional aids and
equipment to enable people to maintain their independence in the home e.g.
shower aids, ramps.

Day Therapy Service: Offers patients a combination of therapies or
clinics in an individually planned program.
A multi disciplinary team works across the in patient and out
patient areas, ensuring appropriate assessment and care planning, the team
includes: -

Referral to the Rehabilitation and Aged Care Service is usually
via the General Practitioner or a health professional in another hospital. The visiting hours for the ward are 8.00 am to 8.00 pm each day
however it is requested that visitors do visit at meal times, 12.00-12.30 and
5.00pm to 6.00pm. For further information on any of the services or referral
