AHS Physiotherapists can provide treatment for a wide range of physical conditions which compromise the mobility, independence or comfort of people of all ages in the community.

The Physiotherapy Outpatients Service is located in the Carl Streich Rehabilitation Unit.

There is a drop-off point at the entrance, and parking is available in the central and lower car parks. Disabled parking spaces are available close to all entrances.

Referrals must be made by a medical practitioner or an allied health professional.

Referrals may be mailed to:- AHS Physiotherapy Outpatient Service, PO Box 460, Armadale WA 6992, or faxed to:- AHS Physiotherapy Outpatients Service Fax No 9391 2262
 

Referrals to outpatients physiotherapy will be acknowledged by mail or telephone within 10 days of receipt. An appointment information package will be sent to the patient, who is expected to provide the requested details and return the information to the Outpatients physiotherapy assistant.

Waiting time for initial appointment will vary according to the current caseload.

Holders of Department of Veterans Affairs cards, some workers compensation cases, and privately insured individuals may be able to access private services without delay.

Urgent appointments are only available with a Medical Practioner’s request.
